As a reminder, we will need up-to-date bloodwork performed within the last 30 days prior to scheduling your dental procedure. It is significantly cheaper for the pet owner to get their pet’s bloodwork performed with their primary veterinarian than through ADCC.

I authorize the release of my pet's medical records to Animal Dental Center of Cypress.

I understand that previous medical records must be emailed to animaldentalcentercypress@gmail.com 24 hours prior to my appointment.

I understand that if my pet's most recent bloodwork results (within 30 days) aren't emailed and received by my pet's scheduled dental, that bloodwork will have to be run at an additional cost.

I understand that my pet must have a minimum of a rabies vaccine and DHPP/FVRCP to be seen in the clinic.
